  6. Affidavit of Attestation to be executed by the members of the Board of Trustees.  The members must attest that they were never involved in the illegal/unauthorized operation of private school;
  7. Favorable indorsement from the Schools Division Office which has jurisdiction over the place where the school will operate.

  4. Certificate of No Pending Case from Legal Unit (Regional Office)
  5. School/Division Office
  6. CSC Form 6 (for approval of RD)

    Additional requirement for Division Chiefs and higher
  7. Special Order designating an Officer-in-Charge (if applicable) – so as not to hamper the day-to-day operations of the Office.

OFFICIAL FOREIGN TRAVEL

  1. Indorsement of the SDS
  2. Travel Authority for Personal Travel (Annex A)
  3. Signed invitation addressed to the requesting party from foreign host
  4. Approved Activity Request and Work and Financial Plan indicating that funds are earmarked for the travel expenses to be incurred
  5. Itinerary of Travel
  6. Written Justification (D.O. No. 43, s. 2022. par. IV.A.3.)
  7. Certificate of No Pending Case from Legal Unit (Regional Office)
  8. School/Division Office
  9. Certification from Accounting Division/Section/Unit that the previous cash advance has been liquidated (if applying for cash advance)

    For Personal Travel, shall be received by Regional Office at least 15 days before departure (fund source: personal/foreign host/SEF)
    For Official Foreign Travel, shall be received by Regional Office at least 35 days before departure (fund source: MOOE/local funds)
